SNS: Is healthcare free in Portugal?

The Serviço Nacional de Saúde (SNS) is Portugal’s tax-funded public healthcare system, which makes healthcare almost completely free. However, there are a few exceptions and small costs to be aware of. 

What is covered by SNS:

  • Essential care: In 2022, Portugal eliminated fees for most services. General doctor visits, specialist appointments, maternity care, hospitalisations, and treatments for serious illnesses are fully covered. However, you may face small co-payments (around 5 to 15 EUR) for certain non-essential visits. So, don’t be caught off guard if you’re asked for a payment.

  • Exempt groups: Children under 18, pregnant women, disabled persons, blood donors, and other groups – according to the Liga Portuguesa Contra as Doenças Reumáticas (LPCDR) – don’t have to pay any medical fees whatsoever. 

What is not covered:

If you haven’t found a service in the basic SNS package, in most cases, you have to pay for it yourself. For example:

  • Prescription medications: Most people pay the full price, however, individuals with chronic illnesses or low incomes can receive medications completely free.

  • Dental and optical care: The SNS does not cover routine eye care, cosmetic surgeries, or standard dental care. 

Tip: Free dental care is provided via a "dental voucher" system to specific groups, such as pregnant women, children up to age 18, etc. 

So, the answer to the question ‘Is healthcare free in Portugal?' is yes, but only partially. To know whether the service is covered or not, you can ask the medical centre directly.

To register for public healthcare in Portugal and access the Serviço Nacional de Saúde (SNS), you first should become a legal resident. If you are, follow these steps (as of March 2026):

  • Step 1: Register with Social Security 

Your employer will enrol you in the social security system automatically. If you are self-employed, you should register with the Social Security Institute directly. 

  1. Your ID or a valid passport.

  2. Valid residence permit.

  3. Portuguese Tax Identification Number (NIF).

Lastly, a healthcare user number (Número de Utente do SNS) is issued. The number grants access to public medical assistance, so don’t lose it. After registering, you will also be assigned a family doctor at your local health centre.

The semi-private sector is made up of "health subsystems," schemes for specific professional groups.

  • Who is covered: Exclusively available to certain professions. Examples: ADSE (for public servants and their families), SAMS (for employees in the banking and insurance sectors) and Military and Police Schemes.

  • How it works: Subsystems don’t replace SNS but act as a supplementary safety net.

Private sector: What you should know

About 25% of the Portuguese citizens use private Voluntary Health Insurance (VHI). It allows them to bypass long public waiting lists, ensure access to English-speaking doctors, and obtain coverage for services the SNS excludes, such as routine dental and vision care. If you are looking to use the private sector, there are two distinct ways to access it.

Health insurance (Seguro de Saúde) 

Private insurance provides coverage for hospitalisations, surgeries, and treatments within a network of private hospitals. As of 2026, the basic packages start around 15 to 20 EUR per month and usually cover up to 15,000 EUR in medical expenses. The final amount depends on the company and your insurance terms. These basic plans typically require you to pay a small co-payment for individual consultations and exams. 

Health plans (Plano de Saúde) 

Health plans act more like loyalty or discount cards. You visit a specific private clinic and are offered discounts on consultations and medical exams.
